The Northern Arizona VA Health Care System is looking for a Nurse Practitioner (NP) to join the their team! The position is located in Flagstaff, AZ. The NP will serve as a provider in outpatient Primary Care. The NP will perform and record history and physical examinations and records data from clinic visits. The duties of the position include but are not limited to: Ongoing, continuous care of one or more assigned panel(s) of Veterans Utilizing all available tools to enable effective and efficient identification and interventions of individual patients Ensuring appropriate evaluation and access is provided to patients assigned to the patient panel Functioning at the full extent of the team member's relevant clinical privileges, credentials, scopes of practice, elements of practice, certification, or other facility approved documentation of competency Participating in team performance improvements and sustainment activities to optimize team efficiency and care delivery to patients Implementing primary care operations management processes, as appropriate Managing communications and facilitating safe transitions of patients between PACT's site of care and other health care settings, using formal and informal methods Providing health education, and health coaching on wellness, disease prevention, chronic care management, and self-management skills to patients and personal support persons Engaging patient in using health care, encouraging patients to engage personal support persons, receiving input from patient and personal support persons regarding VA care Using formal and informal communicators that are respectful, effective, timely, and bi-directional with all team members Integrated Medicine PACT (IMPACT) Pain Rehabilitation Program specific duties include but are not limited to: Practice on integrated medicine team which consists of Provider, RN, LPN, MSA, Psychologist, Integrative Medicine Specialist (Chiropractor); Pharmacist, Dietician, and Rec Therapist Once weekly team meetings (1 hour) Joint/Trigger Point injections Opiate management Review and respond to pain e-consults Maintain a suboxone waiver and prescribe as indicated Follow Whole Health Principles Follow safe opiate prescribing guidelines Service on Opiate Safety (OS) and Pain Committees Spinal Cord Injury specific duties include but are not limited to: Use of consults, that are SCI specific Coordinates any needed consultative care for enrolled SCI Veterans Coordinates Veteran care with designated SCI Centers of Care Exhibits competency in the management of medical issues more prevalent in SCI populations including but not limited to: postural hypotension, autonomic dysreflexia, neurogenic bladder and bowel, impaired cough and respiratory complications, sleep disordered breathing, neurogenic complications of spasticity and chronic pain, management of wounds/pressure ulcers, and integument and musculoskeletal disturbances Participates in Continued Medical Education opportunities and trainings pertaining to the care and management of spinal cord injured Veterans Work Schedule: Full-Time. This position potentially requires flexibility in schedule and assignments, as this is a 24/7 hospital. Some positions require rotation of tours or indefinite work on non-administrative tours as established by the immediate supervisor. Not all duties may be listed, so other duties may be assigned.
